Steps for online RMA request:

1. Fill out the RMA Request Form.

Note: Only 1 product per RMA form.
To return a product to us you must complete all required fields on the RMA form.

2. Fax us the purchase receipt along with your name and contact informaion.

RMA Approval

On approval of the form data and proof of purchase we will issue an RMA for the returned product.
